All medical plans contracting with the State of Colorado submit provider data electronically to Medical QuestTM. While every attempt is made to provide the most accurate information available, you should contact the appropriate carrier listed below to assure that your provider is participating in a network. If there is any conflict between the information contained on this Website and the contracts between providers and plans, the contracts will prevail.
Some health plans use a Personal Care Network (PCN), consisting of a Primary Care Physician (PCP), specialists, and hospital to which a PCP refers. You will need to call your health plan to find out if PCNs are in effect. Where a PCN is in effect, the Primary Care Physician must use only a select group of specialty care providers for referrals. Refer all questions about PCPs and/or PCNs to the carriers listed below.
